Rates & Insurance

 

 

Payment Information:

Payment is expected at the time of the scheduled appointment.  I accept cash, checks, and credit cards (Visa/Mastercard/AmEx/Discover).  Checks should be made payable to Saadia Alizai-Cowan, MD.  Late fees may appy for any outstanding balance.  There will be a processing charge for returned checks. Accounts with unpaid balances may be referred to collections

I do not accept insurance, but I will provide a billing receipt that will describe the type of appointment and diagnosis.  This form can be submitted to your insurance company for reimbursement.  Your insurance policy is a contract between you, the employer, and the insurance company.  Therefore, you are expected to interact with your insurance company for pre-authorization (if necessary) and to initiate/maintain reimbursement. 

If you request me to complete insurance forms, I will return them to you if you would like to see what information is being shared.  I encourage you to find out from your insurance company about your rights of privacy.  Insurance paperwork and treatment plans will be completed free of charge.  Any reports, beyond basic insurance forms, that are requested will be prepared for a fee, based on the amount of time required.

I have opted out of Medicare and do not work with TriCare.  Therefore, those individuals with Medicare as a primary or secondary provider who wish to see me must sign an agreement that states that they will not submit claims to Medicare. Due to the policies of TriCare, I am not able to work with patients covered by TriCare.

 

2023 Fee Schedule: 

Fees will be increased annually. The rates for the following year will be listed here no later than December 1st of the preceding year.

Initial Appointment, 90-120 Minutes - $695

60-75 Minute Appointment - $430

40-50 Minute Appointment - $335

15-25 Minute Appointment - $235  

Report Preparation - $330/hour, billed in 15 minute increments

Phone Calls - There is no charge for brief phone calls

 

Cancelation Policy: 

Forty-eight hour advance notice is required for canceled appointments.  You may leave a message to cancel the appointment.  Sessions missed without a 48-hour notice will be charged the full fee.  Insurance companies do not reimburse for missed appointments.   If you miss an appointment without adequate notice, I may request that you give me a credit card number, which will be used in case of future, missed appointments.
